What is Chronic Disease?

Chronic diseases are prolonged conditions that requires constant monitoring and treatment. Diabetes, asthma and congestive heart failure are examples of chronic diseases.

It affect millions of individuals worldwide and place a substantial burden on society through lost productivity and increased health care costs.

Chronic illness has a profound effect on the physical, emotional and mental well-being of individuals; often making it difficult to carry on with daily routines and relationships. However, in many cases, deterioration in health can be minimized by good and systematic care.

What is Chronic Disease Management?

Chronic disease management (CDM) is a systematic approach to improving health care for people with chronic disease. It is a broad set of standards, best practices and guidelines designed for treatment and prevention.

Health care can be delivered more effectively and efficiently if patients with chronic diseases take an active role in their own care; and providers are supported with the necessary resources and expertise to better assist their patients in managing their illness.

The systematic care of patients are performed by a group of providers, which includes the general practitioner, specialists, staff doctors and pharmacists. Within this group, efficient sharing of information while maintaining privacy and security of patient data is important in providing proper and effective care.

Our strategy

Our strategy with Caresharing is to develop a suite of applications in support for CDM, in areas like delivery of care, monitoring, communication and information sharing. We aspire to achieve our goal through the latest in both Internet and Web technologies.

The Caresharing applications allows convenient and secure access to information through the Web, using universally available web browsers. Care providers will be able to import, view, manage data and collaborate with each other through their workstations. Likewise, patients will be able to review, respond and participate in the process.

To achieve a seamless and effective user experience, our applications are designed with each of the provider's needs in mind. We adopt the latest innovation in web user-interface designs; following the "same page" approach where data are presented in a context-sensitive perspective. Relevant data are intuitively organized on the screen for quick reference and lookup, providing inputs to make a timely and correct decision. Working with data is also done in an efficient manner, loading and updating them where needed, instead of loading the entire web page every time.

We employ some of the best technologies available today, like the Ruby On Rails framework and advanced Javascript techniques like AJAX. We also follow the open industry standards like OpenID, XHTML, CSS and REST.

Data is private, secure and ready when needed

From the beginning, we keep in mind that access to data is secure and authorized, which is based on the multi-tiered patient-provider relationships. Access is also transparent, where users can verify who has access and to which data.

For security, we use the industry standards in developing and hosting our applications, using SSL, AES encryption, automatic self-renewing web sessions, and the latest in system intrusion detection and prevention. We run them on bare stripped-to-minimum FreeBSD and Linux virtual servers, then host them on one of Europe's best VPS hosting provider.
